Meet Our Community Nurses
Several community nurses (formerly known as District Nurses) work at Brannam, although they are employed by the Primary Care Trust (PCT). They visit patients at home to teach independence through self-care, provide post-surgery care, perform general nursing procedures and investigations, provide care for terminally ill patients, assess patients for care needs, loan equipment and care for wounds and leg ulcers.
Referrals usually come from a doctor or hospital but can be made by patients themselves, carers or family members.
We work closely with our team of Community Nurses as they provide essential nursing care to patients in their homes both in and out of hours.
