Patient Representation Group

Would you like to have your say about the services at Brannam Medical Centre?

We aim to maintain a positive and constructive relationship with our patients, to ensure the practice remains accountable and responsive to all its patients needs.  Our Patient Groups form an important role in informing, supporting and developing the services we provide at Brannam Medical Centre.

We are always looking for new members to get involved and make a difference, and it is our aim to encourage patients from as broad a spectrum as possible to get a truly representative sample. We need young people, workers, carers, retirees, people with long term conditions and people from non-British ethnic groups.

There are two options, depending on the level of involvement you are able to offer:

PRG

Our Virtual Patient Forum

We ask the members of this group some questions from time to time, such as your views on the availability of appointments or the quality of the care you received.  We contact you via e-mail and keep our surveys succinct so it shouldn’t take too much of your time.  This might be relevant to those patients who would like to be involved but may not be able to commit the time needed to attend meetings.

If you are happy for us to contact you occasionally by email please click the link opposite to open the form and complete all the fields:

Our Patient Reference Group (PRG)

This is a small group of patients who volunteer to help ensure that Brannam Medical Centre provides the services patients need. The group meet quarterly to discuss planned health service developments, health promotion initiatives and to provide the opportunity to raise any issues which are of concern and/or will help to improve the standard of care offered by the practice. Members may also become involved in Practice patient initiatives.

If you are interested in taking on a role in the group and would like to know more, please download and complete our application form and return it to the surgery.  Alternatively, you can phone reception on 01271 329004 and speak to a Reception Supervisor or complete a patient triage, choosing option 'admin query' followed by ' something else'. Please note, membership to this group is limited.
